Emergency Care program

Create a pilot program that provides patient history to emergency response teams to help guide initial treatment.

How it’d work: Analyze Humana’s database and invite identified populations to create PHRs through Vital Data Technology. Vital Data Technology’s PHRs are directly linked to emergency care services, and hence invaluable at the point of care in an emergency/life-threatening situation. Vital Data Technology allows the emergency personnel to make the most well-informed decisions, based on information in the PHRs. As a pilot program, Humana could identify specific groups of population, including:

o Seniors diagnosed with chronic diseases
o Members who have used an ER more than twice in the past year

Humana could also set up automatic reminders for members to subscribe to MyVitalData - every time there is an incidence of a member using emergency services.
