Better Informed Care Program

Create a joint pilot program that ensures higher quality of care in both - emergency and non-emergency health situations.

How it’d work: Explore a three-way partnership between SafeMed, Vital Data Technology and Humana. Humana, for example, could invite a pilot member base to sign up for PHRs through Vital Data Technology, hence assuring better-informed care in an emergency situation. SafeMed could then use the PHRs of pilot members as the foundation and offer its cutting edge analytics to primary care physicians of the pilot members who sign up for MyVitalData. SafeMed solutions would hence lead to better clinical decision support and higher quality of care. Humana could start the pilot with members with specific chronic conditions, and long-term histories with specific PCPs.

Close, but without incentives why would I use a PHR, why woudl a doctor change his practice behavior and why whould I as a patient change my behavior? Just having the tool or data is not the solution. I think we have all the data we need. Accessing the data, creating actionable data and using resources to act on the data is what is missing. If you bundled that with incentives you might have the foundation for a good solution.
